About Us
The Primary Care Coordinator serves as a shared resource for the collaboration structure in Sörmland. This structure forms the foundation for cooperation and development regarding social services and healthcare between municipalities, primary care centers, and hospitals. The position belongs organizationally to the healthcare leadership, with the Division Manager for Primary Care as the immediate supervisor. We are now seeking someone who wants to work as a Primary Care Coordinator with a commitment to primary care issues throughout the county. The coordinators collaborate to create conditions for good and accessible care. It is an independent role within the collaboration between municipalities and the region, offering you the opportunity to make an impact!
Your Responsibilities
Within the primary care collaboration structure, principals and organizations work together to meet the population's needs for healthcare and care services. The foundation is the collaboration between municipalities, primary care centers, and hospitals. The work is flexible and varied, requiring you to be prepared to take your own initiatives and make independent decisions.
The Primary Care Coordinators work together on a county-wide basis, driving joint assignments and projects. These include developing proposals for county-wide routines and agreements within Primary Care and collaboration, as well as driving processes such as proactive measures to avoid unnecessary inpatient care. Region Sörmland and the region's municipalities have agreed on a vision for Primary Care, with the guiding words: Together – through participation and coordination with the individual at the center; Close – through accessibility and continuity; and Good health, care, and support – through health-promoting, preventive, and rehabilitative measures.
An important part of the role is to develop and maintain close relationships with relevant operations, acting as a contact person and information conduit, as well as participating in external-facing work. Experience in the field of Primary Care is an asset. Local work includes participating in working groups within areas such as elderly care, psychiatry, children, and disabilities. The coordinators are responsible for coordinating meetings and facilitating the further development of Primary Care work. The assignment also includes participation in the county-wide steering group (County Steering Group) in Sörmland. Being a Primary Care Coordinator is a highly independent role with excellent opportunities to develop the assignment.
A certain portion of the position is expected to consist of clinical work.
Your Competence
We are looking for someone who is communicative, curious, and eager to make a difference. You have a desire to develop cooperation within and between organizations. You are a person who sees the big picture combined with strong organizational skills.
Requirements for the position include education in one of the healthcare professions requiring a license, with experience in healthcare. The position requires good verbal communication skills and computer literacy. Several years of experience working in healthcare and collaboration with various actors within healthcare in the region and municipalities is an asset, as is knowledge of the healthcare system and collaboration structure in Sörmland. Experience in project management and development work is also considered an asset. A valid driver's license (Class B) is a requirement.

Employment Type
Permanent employment with a start date to be agreed upon. The position can be located in Nyköping, Katrineholm, or Eskilstuna.
